These Terms of Service (“Terms”) are a binding agreement between you and Thrive Homes, LLC (“Thrive Homes,” “we,” or “us”). Thrive Homes provides home modification and renovation services that improve accessibility, including ADA-compliant upgrades. Our website and portal are tools for clients, Managed Care Organizations (MCOs), Medicaid, and subcontractors to request, schedule, and manage projects. While our process is clinically informed, Thrive Homes does not provide healthcare or medical advice, and nothing we offer should be taken as medical treatment.
2. Eligibility and User Responsibilities
The Services are intended for adults who can legally enter into agreements in their jurisdiction. By engaging with Thrive Homes, you confirm that the information you provide is accurate and that you are authorized to submit it. You agree that you will not misuse the website or portal, attempt to disrupt their function, or upload false or harmful content.
You are responsible for:
Ensuring that all information you provide is truthful and complete.
Updating your information when it changes.
Keeping login details secure, if you are provided access to our portal.
3. Information We Collect and Use
In order to deliver our Services, Thrive Homes collects certain information. For clients, this includes names, addresses, contact details, and project-related materials such as photographs or home layouts. For MCO or insurance representatives, this includes contact information necessary for authorizations and billing. For subcontractors and vendors, this includes business and compliance documentation such as W-9 forms, licenses, and proof of insurance.

4. Sharing of Information
We only share information when it is required to provide Services, including:
With MCOs, Medicaid, and insurance providers for authorizations and claims.
With subcontractors and vendors engaged to complete renovation projects.
